Gather Health, a Boston, MA-based company providing a primary care solution for older adults, raised $15M in Series A funding.

Backers included Khosla Ventures, Maverick Ventures, and Commonwealth Care Alliance.

The company intends to use the funds to expand operations and its business reach. Gather Health has opened its first primary care practice in Quincy, MA and will be launching other locations and markets to follow in 2024.

Led by Dr. Brent Asplin, Chief Executive Officer, Gather Health aims to improve lives and communities for older adults by championing socially-supported and holistic primary care as a collective interest. The company links individual with a team of local advocates dedicated to helping them manage their personal health. Its primary care model leverages a provider with a care team approach. EMTs and Guide Partners are hired from and for the communities being served and patients can get access to in-clinic, at-home, or virtual care, depending on their medical and social needs.

Each dedicated care team will consist of a Primary Care Provider; a Guide Partner to provide social support and to help navigate the healthcare system; and a Medical Technician Partner to provide patients with at-home visits. Gather will also offer transportation to patients for medical visits, a neighborhood office with space for classes and social events, and technology-enabled care designed to help patients in the comfort of their homes.
