Solera Health, Inc., a Phoenix, AZ-based technology-enabled health community integrator for chronic disease prevention, raised $3m in Series A funding.
Backers included BlueCross BlueShield Venture Partners (BCBSVP) and Sandbox Industries. In conjunction with the funding, Dan Phillips, Managing Director of Sandbox Industries, will join Solera’s Board.
The company will use the funds to advance its technology platform and scale operations.
Led by Brenda Schmidt, founder and CEO, Solera provides a proprietary SaaS platform that connects a national network of local community organizations and digital solutions for chronic disease prevention programs, including the National Diabetes Prevention Program (DPP), which is led by the Centers for Disease Control and Prevention (CDC).
The company’s platform administers and streamlines DPP referrals, manages reimbursement and payment, and aggregates data and reporting, simplifying enrollment and supporting consumer engagement and choice.
