Clarity Health Services, a Seattle, WA-based provider of care coordination solutions for medical practices and health systems, closed a $6.6m funding round.
The round was led by Columbia Pacific Advisors’ Opportunity Fund, which had already provided $3.0m in 2011.
The company intends to use the funds for sales and development efforts.
Led by Bill Bunker, CEO, Clarity Health Services provides care coordination solutions that enable healthcare providers to manage care transitions.
Its web-based service offers:
– Real-time care coordination and streamlined referral processes
– Standardized workflows and processes that work with existing EMR, HIE or practice management systems, and
– Data and analytics to help users understand key transition patterns.
