Vivify Health (formerly Intuitive Health, Inc.), a Plano, Texas-based provider of a cloud-based Remote Care Management platform, received a funding of undisclosed amount.
Backers included Ascension Health Ventures and Heritage Group. In conjunction with the funding, Victor Kats from AHV and Rock Morphis from Heritage have joined Vivify’s Board of Directors.
The company intends to use the capital to accelerate adoption of its platform.
Founded in 2009 by Eric Rock, Vivify Health provides hospitals, home health agencies, payers and other stakeholders with a cloud-based Remote Care Management platform to accomplish population health objectives, including reducing readmissions, managing chronic diseases, and improving care transitions.
The company’s SaaS platform delivers customized care plans, coaching, educational video content, and interactive video conferencing. It integrates with health information processes and systems such as EMRs, PHRs and HIEs.
