Welkin Health, a San Francisco-based company providing solutions for population health management, closed a $2M seed funding round.
Backers included Rock Health, Asset Management Ventures, Great Oaks VC, iSeed VC, a direct investment from iKang Healthcare Group, and Mayo Clinic.
The company intends to use the funds to expand its care coordination platform to include chronic disease care for hypertension, COPD, and congestive heart failure, and also further develop tools for administrators.
Led by Chase Hensel, CEO, Welkin Health provides a digital health platform for care organizations to manage daily outreach to hundreds of people with chronic illness through telephone, SMS, and smartphone app for patients.
The company has also announced a new partnership with Humana’s Innovation Group, a Louisville, Kentucky-based health and well-being company.
