RightCare Solutions, a Ft. Washington, Pa.-based provider of hospital discharge planning and readmission management solutions, has raised $1.75m in Series A funding.
Backers include Compass Partners and Domain Associates.
The company intends to use the capital to expand the team to further develop their platform and increase early market adoption.
Led by Co-founder & CEO Eric Heil, RightCare Solutions provides a Discharge Decision Support System Platform designed to help hospitals identify and avoid unnecessary readmissions and allow them, insurers, and homecare agencies improve the quality of care for patients, by enabling point-of-care decision support, optimizing workflow, and improving post-acute care transitions.
The company’s D2S2 platform is based on research conducted at the University of Pennsylvania School of Nursing.
